What is your ID? If you are taking this survey by yourself, you can make up your own ID. (Do NOT enter any personal info like social security names or dates of birth, please!) If you are taking this as a student or under the auspices of an organization, the ID is a unique number or letter combination that your teacher or site gives you. If you don't know it, exit now and find out first!

To obtain your report

Save or print your report!

When you finish taking the survey, you can save the report as a PDF file and/or print it out. Please do this in order to maintain a copy for your files. We rely on volunteers to make this survey available at no cost; thus, we cannot respond to requests to retrieve individual reports.

Executive Skills Questionnaire (ESQ)

Everyone has both strengths and challenges in these skills depending on the situation they are in. Be as honest as you can in deciding what your strengths and challenges are. In particular, think about how you handle non-preferred tasks (the kinds of tasks you don't particularly like to do). Please read each item and decide how often you consider it to be a problem for you using the following scale:
